The driver of a green Ford Fiesta, a man from Chepstow, was also injured in the collision and was treated by paramedics at the scene.
The road was closed for around five and a half hours between Lydney and Elton while the emergency services worked at the scene.
One driver, who didn't want to be named, said: "I saw the road was closed and immediately thought the worst.
"That road is so bad, there are accidents all the time. Something needs to be done to stop it.
"I really hope everyone is alright."
Witnesses to the collision who haven't yet spoken to officers are asked to call Gloucestershire police by dialling 101 and quoting incident number 42 of February 27.
