Vickery gets another chance to tame 'The Beast'
With Wales prop Adam Jones joining Scot Euan Murray on the injury list, the former England captain is forced back into action against the man who took him to pieces two weeks ago.
Joe Worsley is another of those brought in, and he feels Vickery will be up for the challenge.
"I have known the guy for years," said flanker Worsley, who makes his Lions Test debut at Ellis Park.
"When you do have a bad game or things go against you, then you are just waiting for the next opportunity to go out there and do something about it.
"I bet he's over the moon that he has got a chance this weekend, rather than stewing all summer.
"There are a lot of tough nuts out there, and he's one of the toughest."
Lions boss Ian McGeechan added: "Phil is a world-class prop and an outstanding tourist.
"He put all his efforts into supporting Adam (Jones) when he got the position for the second Test, and you couldn't have asked for any more.
"Technically, he helped him, working with him, and that's the way he has kept himself very close to what we have been doing. He has been tremendous in the support he has given."
Gethin Jenkins is also injured, meaning Andrew Sheridan comes in on the other side of the scrum, as McGeechan makes wholesale changes.
Both centres, Brian O'Driscoll and Jamie Roberts, are ruled out, with Riki Flutey coming in and Tommy Bowe switching from wing.
Harlequins' Ugo Monye returns after he was dropped following the first Test, with Shane Williams replacing Luke Fitzgerald on the other wing.
Worsley and Martyn Williams come in, meaning only skipper Paul O'Connell, half-backs Mike Phillips and Stephen Jones, and number eight Jamie Heaslip have started all three Tests.
